Effectiveness of an end-of-life family support nursing educational program in neonatal intensive care units

This study evaluated an educational program developed to support nurses' attitudes, knowledge, and skills for end-of-life (EOL) family support in Neonatal Intensive Care Units (NICUs). A quasi-experimental one-group pretest–posttest study design was used. Participants were 46 nurses with EOL care experience working in NICUs or growing care units at tertiary perinatal medical centers in Japan. The primary outcome was nurses' attitudes toward EOL family support, measured using the ELNEC-J CQ Domain X. Secondary outcomes included perceived knowledge, communication skills, and difficulties in providing family support. Attitude scores, including confidence and competence, increased significantly from pre-to post-intervention and at one month (p < 0.001, Friedman test), suggesting that program participation may be associated with more positive self-reported attitudes toward EOL family support. The findings further indicate that program participation may contribute to improvements in NICU nurses’ self-reported attitudes, perceived knowledge, and communication skills, as well as reductions in perceived difficulties. © 2026 Neonatal Nurses Association.
